Although Mihoshi meant well, she had no idea what she was doing. After all, granting wishes is more complicated than it looks. And it's much more boring, which explains why Mihoshi was in a hurry with only this wish and two more to get through and then she was done and she could go back to her normal life!
This didn't bother the Djinni Council who were actually granting the wishes in the least by the way. Their thing was Chaos with a capital X, because things were so much more boring when they stopped moving.
Now, generally speaking, off-the-cuff wishes were considered best by those whose occupation was granting wishes, as those were supposed to most clearly reflect the unconscious desires hidden in the heart.
Which is why, when Nabiki made an off-hand comment that Ranma might switch places with a manga character, Mihoshi used that and whatever was on top of the pile as the actual wish. The Djinni Council reviewed the rather confused mess that Mihoshi sent them, and resolved the matter in an unorthodox manner.
So, say goodbye to Ranma Saotome, and say hello to:
