Ranma couldn't have known this. He was a masterful martial artist, but not exactly an A student in other matters, the latter being the price for the former. Though he may blow it off with the words "School's boring anyway," secretly he knew he was ignorant about many things, some of them potentially important even to a masterful martial artist. It was a price he had started paying before he knew enough to make such a decision, and he continued paying it willingly, now that he understood the sacrifices one had to make for true mastery.
In this case, however, even a complete school education would not have helped Ranma, even though he later blamed himself for not having paid enough attention during Biology or Chemistry lessons, or whatever. He could not possibly have guessed at the effects a herbal concoction, designed to mildly stimulate lactation and hormonal glands, would have when it hit a metabolism that was already being stimulated by a large dose of caffeine.
A fully trained biochemist could not have made a definite prediction, but if one had been asked to provide a list of educated guesses, the top ten possibilities would certainly have included the actual effect Nabiki suffered (apart from lactation), which was...
