It struck a young girl in Juuban who was studying with her friends.

It struck here and there - and girls (and former guys) who were engaged to Ranma in some loose form.

However, in order to accomplish this major task, the Wish was making a little adjustment in time and space. So that:

• the change is NOT retroactive, but the girls see nothing wrong with being Ranma's sex slave. Worse - any new engagements cause the individual engaged to Ranma to be hit by the curse.
• the change IS retroactive. Nobody (except Ranma) remembers the girls as being anything other than his slaves.
• As #2, but the girls are furries or robots or minor genie or something - so it's all perfectly legal.
• While it's new, something in the enchantment hits those who protest Ranma having these girls - adding the critic to the herd.
• some other twisted little option
