Without overwriting base elements of the reality, there were millions of possibilities for completely screwing with the timeline - however certain things pretty much doomed the whole thing to a Bad Ending of one kind or another.
The whole thing started with a Progenitor Type III-A Lifeform, which created a massive number of life-forms based on its akashic and overdimensional patterns. Reflected in the RNA and DNA blueprints, each of the Progenitor types would then spawn entire biospheres based on itself when it combined with a Progenitor Type III-B form. The problem being that once they reached the far end of that development, a set of circumstances to test the existing lifeforms and then erase and start again would occur.
Unless he did something about THAT endgame, some things would end up pretty much in the same toilet.
There WERE ways to do it within those rules, a fair number of the end results involved Shinji Ikari making a choice at a critical moment when he was pretty much a mental wreck.
Another possibility was Rei Ayanami. As she was a mix of Lillith and Yui Ikari, there were things that could be done there. The only problem was that as she was carefully cultivated and watched by Gendo Ikari - if she changed too much she'd be killed and replaced with one of the others unless something was done about that as well.
Misato could have gotten a clue earlier. Ritsuko could have realized how much like her mother she'd become and rebelled against that. Asuka could have done something clever when she'd really needed to be clever.
At the end stage though - no new conceptions - which really should have caused more concern. It had occurred before the Last Angel had made an appearance, that was just when people outside NERV and Gehirn had started noticing.
After some consideration, Toltiir decided to try just affecting ONE character. That one being:
